Are you interested in joining a dynamic team at Rush University Cancer Center? We are seeking an experienced and highly motivated Supportive Oncology Social Worker – Level 2 to support patients engaged with the Bone Marrow Transplant team.

General Summary:

The Supportive Oncology Social Worker – Level 2 is a key member of the Rush University Medical Center community. This Illinois-licensed social worker provides outpatient care for patients engaged with RUSH MD Anderson Cancer Center’s Bone Marrow Transplant (BMT) and Cellular Therapy Service. The social worker will work closely with the BMT psychologist and collaborate with BMT medical providers to support this patient population throughout the transplant process. The social worker provides on-site patient care through a diverse role as a clinician, care manager, and advocate.

Primary responsibilities include managing a caseload of patients and performing biopsychosocial assessments, providing care management services, addressing resource needs, providing patient/caregiver & staff education, and providing crisis interventions when needed. This position is part of the Supportive Oncology team, a multidisciplinary team supporting the Rush Cancer Center, and the social worker works in collaboration with other healthcare team members to meet patients’ psychosocial needs. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with Rush policies and procedures.

Required Job Qualifications:

• Master’s degree in Social Work.
• Licensed Clinical Social Worker (LCSW) in State of Illinois.
• At least 4 years’ post-graduate professional experience in healthcare, care management, mental health and/or social services.
• Clinical experience working with adult populations.

• Strong written, verbal, and analytical communication skills.

Responsibilities:
• Provides social work services to outpatient oncology patients, families, and caregivers engaged with Rush University Medical Center’s Cancer Center.
• Addresses the psychosocial needs of Rush oncology patients who have been referred for social work services.
• Works directly with patients and their families to address complex issues that arise from unmet needs and provide the necessary social work services to help them achieve their care goals.
• Conducts assessments in order to identify patients’ psychosocial needs.
• Develops a psychosocial plan of care and facilitate clinical interventions to meet patients’ needs.
• Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
• Problem-solves with patients, their health care team, and other providers as needed regarding psychosocial needs and care coordination.
• Documents all patient interactions in Epic electronic health records system.
• Provides on-call availability during Monday-Friday business hours as needed to support patients and providers. Provides crisis intervention and conduct safety assessments when needed.
• Collaborates and consults with community partners as appropriate to support the needs of this patient population.
• Maintains relationships with referring Rush providers through consistent and timely communication, and collaborates with providers and teammates as needed to meet patient needs.
• Maintains social work licensure through attendance at professional development trainings, and engage in oncology-specific trainings as able.
• Promotes the Rush Supportive Oncology program and refer patients to department services as appropriate.
• Leverages experience to support team members in case consultation, training, crisis management, etc. as needed.
• Represents the social work team and the Supportive Oncology department at meetings, conferences, or other public engagements on an as-needed basis.
