The Dialysis Access Coordinator RN works closely with transplant surgeons, interventional radiologists and nephrologists, nephrology providers and dialysis unit staff, and other care team members, as well as patients and families to educate and facilitate the placement and use of the optimal patient access option for dialysis treatments. This may include either vascular access or peritoneal dialysis (PD) catheter access.

•Tracks UTP access patients to ensure follow-up appointments are made and rescheduled if missed.
•Facilitates communication and care coordination with hospital dialysis access stakeholders, including nephrology team, interventional radiology, and interventional radiology and others as indicated.
•Communicates with inpatient RN coordinators to track UTP dialysis access patients admitted to the hospital and facilitates follow-up and additional testing and procedures as indicated.
•Documents individualized plans of care related to dialysis access developed by the multidisciplinary dialysis access care team. Facilitates multidisciplinary dialysis access care rounds.
•Provides education to patients and families about dialysis access options, procedures, and care, taking into account specific, physical, psychosocial, and learning needs of each patient/family. Advocates for dialysis catheter avoidance/reduction.
•Facilitates clinic evaluation of dialysis access patients, including collecting care data, collating prior medical records including operative reports and imaging studies, and obtaining reports from dialysis units regarding patients’ status on dialysis, clinical concerns, and dialysis adequacy.
•Able to communicate with surgical providers in a clear and succinct fashion.
•Routing cleaning of ultrasound machine and escalating supply ordering needs related to the ultrasound machine effectively to leadership.
•Assist with office procedures in dialysis access patients, including knowledge of sterile technique and common office procedures. Inventory and re-order supplies as required.
•Excellent documentation skills, including ability to upload images, faxes, and other documents sent from referring providers into the media tab of the electronic medical record and communicates receipt of materials with dialysis providers.
•Facilitates clinic flow, including orderly progression of schedule and continuity of individual patient care.
•Provides education to dialysis units regarding patient-specific dialysis access concerns.
