Summary:
The Clinical Dietitian 1 is responsible for providing medical nutrition therapy for patients at RUSH. Active participation in the treatment process is essential as the Clinical Dietitian interacts with patients and members of the healthcare team. This role will care for patient populations that are frequently unable to meet nutrition requirements orally, requiring enteral nutrition (EN) or parenteral nutrition (PN). This may include, but is not limited to, post-surgical, critically ill, neonatal, pediatric, and geriatric patients. This role will support inpatient and ambulatory care. Summary:

The Community Health Worker (CHW) is responsible for operationalizing Rush’s social determinant of health and chronic disease strategy within the community with our Center to Transform Health and Housing team. The CHW will be a liaison to community partners, and conduct health and/or social determinant screenings, provide telephonic and in person navigation services and self-management support and in the Rush clinical settings both onsite and in the community. The CHW will also assist with different community initiatives such as health programming or other health promotion efforts as needed. The CHW will also assist with elevating the voice of those with lived experience in homelessness, organize and lead Lived Experience integration initiatives and be a co-designer of this work. It is preferred that this CHW have lived experience of homelessness and/or has faced housing insecurity to align with those served within Center to Transform Health and Housing. This position is grant funded through February 2027 with the goal of finding additional funding to continue the role.
