Rush University Medical Center, a nationally recognized academic medical center and the teaching hospital for the Rush Medical and Nursing Colleges, is seeking an experienced Advanced Practice Provider (PA or APRN)to join GIM Rush Connect Primary Care—an innovative, hybrid care model based at Rush Oak Park Hospital. The APP will be a key clinical leader in delivering team-based, tech-enabled, relationship-centered care to a dedicated patient panel.

About GIM Rush Connect Primary Care

Rush Connect Primary Care is redesigning how patients access and experience primary care. The model blends the flexibility of virtual care with the engagement of in person visits to expand reach, strengthen relationships, and improve health outcomes. Care is delivered by a cohesive multidisciplinary team and supported by digital tools, evidence-based workflows, and outcome-driven metrics.

Model Highlights:

  • Hybrid Work Structure: Three (3) virtual care days (~9 hours/day) and one (1) in-person clinic day (~12 hours) per week, supported by standardized operational workflows.
  • Panel-Based Team Care: Shared patient panels with a physician, APP, RN, MA, social worker, and pharmacist enable comprehensive, relationship-based care.
  • Proactive Population Management: MD/DO/APPs lead chronic care management (CCM), remote patient monitoring (RPM), and preventive care workflows across virtual and in-person settings.
  • Technology Integration: Use of Epic, care management platforms, team messaging, and clinical decision support tools to enhance efficiency and quality.
Key Responsibilities

  • Provide direct patient care during both virtual and in-person clinic days, including new and follow-up visits, chronic disease management, results review, and proactive outreach.

  • Conduct and document evidence-based care plans in Epic, leveraging technology to streamline tasks and patient communication.

  • Engage in team huddles, quality reviews, and structured improvement efforts.

  • Close care gaps through targeted outreach, preventive services, and multidisciplinary collaboration.

  • Participate in pilot workflows, technology testing, and innovation cycles within Rush Connect.

The Advanced Practice Provider (APP)is a licensed physician assistant (PA) or advanced practice registered nurse (APRN) who functions as a member of the health care team to provide high quality, comprehensive acute and/or routine medical/surgical care of Rush patients within the scope of practice granted in accordance with the Rush University Medical Center medical staff bylaws. The APP will promote collaboration between multiple disciplines to ensure cost effective and quality patient care. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with the policies, procedures, guidelines of Rush, including the Rush Medical Group Customer Service standards. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards.

Position Summary:

The Advanced Practice Provider (APP) will provider all aspects of care including acute care, well care and chronic care in the urgent care setting for patients of all ages.

    Position Requirements:

    Nurse Practitioner:

    • Current licensure from the State of Illinois as a Registered Professional Nurse and Advanced Practice Nurse
    • Master of Science in Nursing or Doctorate of Nursing practice with clinical specialty appropriate to position
    • Current certification by American Nurse Credentialing Center (ANCC) or American Academy of Nurse Practitioner (AANP)

    • Maintains State of Illinois and Federal DEA license

    • Must maintain Rush University Medical Center (RUMC), Rush Oak Park Hospital (ROPH), and Rush Copley Medical Center (RCMC) clinical privileges, as applicable.
    • CPR, ACLS and PALS certifications.
    • Excellent communicator
    • Experience treating patients of all ages
    • Procedural experience (simple laceration repair, I&D)

    Physician Assistant:

    • Current licensure from the State of Illinois as a Physician Assistant/Associate
    • Successful completion of an accredited Physician Assistant/Associate Program
    • Current certification from National Commission on Certification of Physician Assistants

Rush offers competitive total compensation that includes base pay, benefits, and other incentive programs for our employees. The total pay range shown reflects the wide range of factors that are considered in making compensation decisions including, but not limited to, specialty; relevant experience and training; and other business and organizational factors. This total pay range is for a full-time, 1.0 FTE employee, and includes any incentive payments that may be applicable to this role. Additional compensation may include premium pay rates, sign-on bonuses, and relocation allowances. We also offer a comprehensive benefits package.

Pay Range: $100,651 - $160,284

6.Upon decision of patient’s admission, has the knowledge and skill to perform the admission notification (NOA) process which is a required communication with the patient’s payer to ensure that the payment for patient’s inpatient stay is secured.
7.Appropriately informs the patients of hospital policies that govern the revenue cycle. Minimizes the potential financial risk of patients accounts by discussing with the patient and/or guarantor their financial responsibility for upcoming visits/procedures, past due balances and referral requirements. Offers options and negotiates acceptable resolution of estimated patient balance.
8.Receives and properly responds to, or directs telephone inquiries from patients, payers, physicians and their staff, internal department and other persons and entities.
