Summary
The Metropolitan Chicago Breast Cancer Task Force is a not-for-profit organization formed in response to the large racial disparity in breast cancer mortality in Chicago. The Task Force is working to eliminate disparities in breast cancer outcomes and improve breast health for all Chicago area women through a multi-faceted approach that involves initiatives in policy, advocacy, outreach, education, navigation and quality improvement in medical care. The Community Navigator will be involved in the implementation and expansion of the Community Health program, including outreach, navigation and other initiatives as they are developed. Responsibilities
1.Community Based Navigation (70% of Time).

Summary:
The Care Management Navigator I is part of a care management team that is responsible for promoting effective resource utilization to achieve an optimal clinical outcome. The Care Management Navigator applies a collaborative approach in working with physicians, nurses, social workers, peers, leadership, community providers, payers, internal/external agencies and patients. As part of the larger Division of Care Management, the Navigator will provide supportive services to one or more areas within the Division. The Care Management Navigator may contribute to transitional care planning, utilization review, and/or service line growth & outreach efforts for a group of patients from a patient list. Responsibilities:
1. Daily monitoring of a list of inpatient discharges, ER discharges, and referrals.
2. Provide outreach for all patients due for follow up and new patient appointments according to policy and procedures.
3. Prepare and deliver routine correspondence/medical records to referring agencies and primary care providers in accordance with policy.
4. Document all correspondence in electronic medical records as necessary.
5. Liaison to multiple departments, agencies and provider offices.
6. Participate in meetings as assigned.
7. Conduct outreach and complete assessments as necessary to identify individuals at risk for adverse health outcomes including timely Health Risk Assessments

13. Works with CM and Patient Access to ensure hospital compliance with federal mandates.
14. Coordinates post discharge care transition referral communication. In collaboration with the case manager, provides referral and follow-up contacts to agencies, facilities and patients/families to support safe, effective care transitions.
15. Identifies issues, resolves when able, triages to the appropriate level.
16. Provides designated support for functions including payer approval/certification communication, denial appeals, and/or level of care management processes. In accordance with the case management external faxing policy and HIPPA guidelines, communicates clinical information/PHI to payers to ensure certification of inpatient hospital stays via secure e-fax.

Qualified candidates must possess a strong commitment to patient care, education, and research. Candidates must have completed a Pediatric Otolaryngology Fellowship, be board certified/board eligible and be eligible for faculty appointment at the Assistant Professor level or above. The candidate will have an opportunity to build a robust clinical practice encompassing all aspects of pediatric otolaryngology, to teach residents and medical students, and to develop a successful research program. The position offers the opportunity to expand this highly ranked program at the Rush main campus and regional sites that will help support the growth of the Section of Pediatric Otolaryngology. Candidates would take part in robust multidisciplinary clinical programs, including Pediatric Aerodigestive Program and Craniofacial Center, while also having the opportunity to grow clinical programs, including airway reconstruction and head and neck masses. The Section supports a busy, 72-bed Level IV NICU (highest IL designation), an 18-bed PICU with 24/7 coverage by pediatric intensivists, and a general pediatric unit at the main campus.
The Otolaryngology department has 21 faculty, 8 APPs, and 11 audiologists that provide comprehensive array of otolaryngology services at the main campus and 6 regional sites. The department is planning to add 3 additional faculty over the next year and two additional regional sites to support clinical expansion. The program is currently ranked #28 on US News and World Report and has a highly competitive residency program with 3 residents/year and fellowships in Advanced Rhinology and Facial Plastic and Reconstructive Surgery.

The Metropolitan Chicago Breast Cancer Task Force is a not-for-profit organization formed in response to the large racial disparity in breast cancer mortality in Chicago. The Task Force is working to eliminate disparities in breast cancer outcomes and improve breast health for all Chicago area women through a multi-faceted approach that involves initiatives in policy, advocacy, outreach, education, navigation and quality improvement in medical care. This is a fully clinical, outpatient position with an academic appointment at the Assistant or Associate Professor level, depending on experience and qualifications. The role is focused on providing comprehensive palliative care services to cancer patients at Rush University Medical Center, Rush Copley, and Rush Lisle.
The selected candidate will work as part of an integrated cancer palliative team that currently includes physicians and Advanced Practice Providers (APPs), with plans for expansion. Our goal is to enhance the quality of life for patients with cancer by addressing symptom management, goals of care, and complex decision-making in a highly coordinated setting.

11. Triages patient phone calls for acute patient issues and counsels accordingly.

Principal Duties and Responsibilities

Qualified candidates must possess a strong commitment to patient care, education, and research. Candidates must have completed a Pediatric Otolaryngology Fellowship, be board certified/board eligible and be eligible for faculty appointment at the Assistant Professor level or above. The candidate will have an opportunity to build a robust clinical practice encompassing all aspects of pediatric otolaryngology, to teach residents and medical students, and to develop a successful research program. The position offers the opportunity to expand this highly ranked program at the Rush main campus and regional sites that will help support the growth of the Section of Pediatric Otolaryngology. Candidates would take part in robust multidisciplinary clinical programs, including Pediatric Aerodigestive Program and Craniofacial Center, while also having the opportunity to grow clinical programs, including airway reconstruction and head and neck masses. The Section supports a busy, 72-bed Level IV NICU (highest IL designation), an 18-bed PICU with 24/7 coverage by pediatric intensivists, and a general pediatric unit at the main campus.
The Otolaryngology department has 21 faculty, 8 APPs, and 11 audiologists that provide comprehensive array of otolaryngology services at the main campus and 6 regional sites. The department is planning to add 3 additional faculty over the next year and two additional regional sites to support clinical expansion. The program is currently ranked #28 on US News and World Report and has a highly competitive residency program with 3 residents/year and fellowships in Advanced Rhinology and Facial Plastic and Reconstructive Surgery.
