      Summary:
      The Care Management Navigator II is part of a care management team that is responsible for promoting effective resource utilization to achieve an optimal clinical outcome. This position works collaboratively with physicians, nurses, social workers, peers, leadership, community providers, payers, internal/external agencies and patients to provide transitional care planning, utilization review, and/or service line growth & outreach efforts for a group of patients from a patient list. This role includes scheduling procedures and outpatient provider appointments as part of the responsibilities. Insurance prior authorization will also be an expectation. In addition, this role will share knowledge and education to empower patients and improve their ability to navigate services and systems.       Required Job Qualifications:
      •High School diploma or GED and 3 or more year of experience in a healthcare or hospital ambulatory related setting.
      •2 years of navigator or a combination of a navigator and care coordinator experience with Comprehensive knowledge of multiple aspects of the program to provide coverage as needed for missing navigators
      •2 years of experience with electronic medical records
      •Demonstrated knowledge and/or experience with case management-related service functions such as: patient interviews, discharge planning/social service basic assessment and referral processes, community resources, finance and payer communication processes and information systems.

      Summary:
      The Medical Staff Quality & Performance Specialist provides process coordination and applies data analytics knowledge to assure that the Professional Practice evaluation process is effective and accurate in evaluating issues related to professional credentialing, conduct and practice. The Medical Staff Quality & Performance Specialist is responsible for collecting data, quality control of information, and statistical analysis of data for use in the OPPE/FPPE process. They will collect and analyze data, interpret results, and submit reports and recommendations to the leadership team as well as interpret results against set benchmarks following analysis and corresponding reports.       Requirements: 

      • Bachelor’s degree. Focus in Healthcare and/or healthcare quality analytics preferred.
      • Four (4) years of experience coordinating and monitoring quality and performance, preferably in an acute care (hospital) setting.
      • Experience with Ongoing Professional Practice Evaluation (OPPE) and Focused Professional Practice Evaluation (FPPE) programs strongly preferred.

           Convenient Care – Lead Advanced Practice Provider

      •Rush Copley Convenient Care serves walk-in patients with minor illnesses and acute injuries, performs school and sports physicals and provides immunizations. On site diagnostic services include Clia waived lab testing, X-ray and CT.  

      •Rush Copley Convenient Care has 7 sites and is open 7 days a week (including holidays) from 7:45am-7:30pm. 

      •The Lead APP role will be 0.80FTE clinical and 0.20FTE administrative and will supervise a team of approximately 20 APPs. 

      •In addition to clinical responsibilities, the Lead will be responsible for creating call schedules, interviewing, evaluations, team development and recruitment.

       •Candidate must possess strong clinical skills, as well as excellent interpersonal and leadership skills. Preferred Qualifications: 

      •Position requires 3+ years experience as an APP with preferred experience in urgent care or emergency medicine 

      The Advanced Practice Provider (APP) is a licensed physician assistant (PA) or advanced practice registered nurse (APRN) who functions as a member of the health care team to provide high quality, comprehensive acute and/or routine medical/surgical care of Rush patients within the scope of practice granted in accordance with the Rush University Medical Center medical staff bylaws.       Job Duties: Rush University Medical Center seeks a Performance Improvement Consultant in Chicago, IL to identify, coordinate, and lead improvement initiatives. Use methodologies including measurement, analysis, development of improvement plan, and the reporting of project results. Provide consultation regarding best practices with emphasis on process reengineering and systemic implementation. Develop measurable end state vision for performance improvement using tools such as lean six sigma, statistical significance, and operational control limits. Utilize change management principles and partner with process owners to facilitate successful project pilots and implementation. Create, monitor, and report metrics/dashboards related to improvement activities. Partner with analysts and auditors to extract and analyze data. Serve as liaison with key stakeholders on tasks, deliverables, and issues regarding improvement initiatives. Provide training and education regarding performance improvement methodology. Maintain knowledge of current trends with the performance improvement community. Collaborate with medical staff and hospital departments to develop standardized care paths and/or standing orders and policies for identified performance improvement initiatives based on best practice and national benchmark data. Review literature and comparative benchmarking data to identify best practices and specific areas of opportunity by focusing improvement initiatives on the effective management of the cost and quality of patient care. Perform detailed root cause analysis of adverse events and near misses or other identified improvement initiatives. Develop and present data including executive summary reports, balanced score cards, and other reporting formats via committee structures as assigned.       Minimum Requirements: Master’s degree in Healthcare Management, Public Health, or a related field of study and 2 years of experience in performance improvement consulting.     Responsibilities:
    1.Responsible for the design, build, implementation, and support of pharmacy systems, including but not limited to Epic, DoseEdge, Alaris, Pyxis and RIVA.
    2.Participates in multidisciplinary meetings including but not limited to, Clinical Decision Support, Medication Safety, Pharmacy and Therapeutics, Pharmacy-Nursing, Anticoagulation Committee, Drug Utilization Evaluation Committee, and Chemotherapy Sub-Committee to maintain and enhance Epic functionality.
    3.Collaborates effectively with the department of Information Technology (IT) and pharmacy informatics team.
    4.Maintain appropriate working relationship with pharmacy staff, managers, and leaders.
    5.Evaluates code changes prior to implementation and participates in integrated testing scenarios as necessary.
    6.Responsible for staffing roles including, but not limited to: IT/Epic Willow on-call, pharmacist staffing shifts, weekends and holidays.
    7.Develops and utilizes reports from various systems to optimize workflows and troubleshoot issues.
    8.Collaborates with Patient Care Finance on questions and issues related to charging in Epic.
    9.Collaborates with the Budget and Internal Audit departments on implementing new costs on a bi-annual basis.
    10.Responsible for adhering to department policies and procedures.
    11.Must demonstrate understanding of interfaces, charge capture, revenue stream, 340B, purchasing, and pricing.
    12.Consistently works towards the department mission and vision, along with the strategic plan devised by the Corporate Director of Pharmacy.
    13.Required to work with pharmacy residents and students for training and precepting purposes.
    14.Demonstrates responsibility and accountability for all aspects of professional practice.
    15.Consistently strives to improve liaisons with other professionals and departments.

    Summary:
    This position is responsible for performing all assigned laboratory testing and reporting for the testing area. This position requires teamwork to achieve common goals and meet deadlines to provide quality care to clients and patients. Demonstrates proficiency in identifying and resolving process issues that affect the laboratory's ability to perform laboratory testing.     Certification from a nationally recognized certifying agency is required. Some laboratory disciplines may require more stringent certification criteria.
    Acceptable credentials (can vary depending on the laboratory section):
    •Medical Laboratory Scientist (MLS) via the American Society for Clinical Pathology (ASCP)
    •Medical Technologist (MT) via the American Medical Technologists (AMT)
    •Blood Bank (BB) via the ASCP
    •Chemistry (C) via the ASCP
    •Cytogenetics (CG) via the ASCP
    •Cytotechnologist (CT) via the ASCP
    •Hematology (H) via the ASCP
    •Microbiology (M) via the ASCP
    •Molecular Biology (MB) via the ASCP
    •Specialist in Cytometry (SCYM) via the ASCP

    •Medical Laboratory Technician (MLT) via AACC or ASCP with a minimum of 7 contiguous year's laboratory-related experience.
    Experience:
    •Bachelor’s Degree with certification, and a minimum of 24 contiguous months of high-complexity clinical laboratory experience required.
    •Associate’s degree and/or education/work experience meeting the qualifications of high complexity testing personnel as defined by CLIA 42 CFR 493 1489; and with MLT or other certification; and 7 contiguous years of high-complexity clinical laboratory experience required.
    CLIA QUALIFICATION FOR TESTING:
    Employees in this job classification are qualified to perform moderate and high complexity testing. Employees' education and/or laboratory experience meet the requirements delineated in the Clinical Laboratory Improvement Amendment of 1996.
    Physical Demands:
    Able to work in fast pace, often stressful, computerized environment. Able to lift 20 lbs and perform repetitive motions. Works in laboratory with exposure to potentially infectious and hazardous materials.

    Responsibilities:
    Advanced Technical Duties:
    •Demonstrates proficiency in all duties as defined in the role of Medical Technologist I.
    •Demonstrates proficiency in identifying and resolving process issues that affect the laboratory's ability to perform laboratory testing
    •Acts as a resource for identifying and solving quality control issues and escalating these when appropriate.
    •Demonstrates troubleshooting proficiency and therefore rarely needs to seek additional help.
    •Required to prepare and present a minimum of 15 minutes of continuing education within the department.
    •Required to identify and participate in process improvement projects.
    •Ensures that sufficient reagents and supplies necessary for test performance are available. Notifies supervisory staff when minimum inventory level is encountered.
    •Serves as an educator and trainer for students and new employees
    •Recognizes when changes in the workflow are needed by adjusting work to incorporate STAT tests or fluctuations in work volume
    •Serves as the owner of a test or process to ensure performance excels minimum quality requirements
    Job Components
    Performs Laboratory Testing
    Utilizes required Personal Protective Equipment (PPE) as defined by the lab area. Labels and processes specimens properly. Performs all laboratory procedures efficiently and effectively with minimal supervision once trained. All testing must be done following Rush-approved Policies and Procedures. Acts as a technical resource for the testing area.
    Quality Control and Preventative Maintenance
    Performs required quality control (QC) procedures and preventative maintenance, including corrective action and documentation, for all procedures and instrumentation. Recognizes QC problems or discrepancies and attempts to determine the cause of the problem. Notifies the appropriate supervisor or designee regarding QC problems or discrepancies according to the established procedure. Acts as a resource for identifying and solving quality control issues and escalating these when appropriate.
    Troubleshooting
    Performs basic and advanced troubleshooting of equipment and processes with minimal assistance, documents action and notifies appropriate personnel, for example, supervisors, of problems all of the time. Demonstrates troubleshooting proficiency and therefore rarely needs to seek additional help.
    Organization of Work Area
    Keeps work area neat, clean, organized, and well-stocked at all times. Disinfect area at the beginning and end of each shift.
    Specimen Acceptability
    Accepts no inappropriate specimens. Notifies the appropriate person of specimens that must be drawn or collected again and documents the notification.
    Turnaround Time Expectations
    Meets turnaround time expectations by prioritizing and organizing the workload for the shift. Monitors the pending list periodically to ensure turnaround time of all samples.
    Time Management
    Utilizes time effectively and seeks out additional job duties when shift tasks are complete. Completes work without incurring overtime. Assists fellow workers in finishing shift tasks. Adheres to the defined allotted time for breaks and lunch
    Continuing Education
    Required to obtain and document 12 hours of laboratory continuing education per year. Required to prepare and present a minimum if 15 minutes of continuing education within the department.
    Special Projects
    Accepts and/or volunteers for method evaluations and new procedure development or other special projects and completes them within an agreed-upon time frame. Participates in procedure implementation and training of new staff and students. Required to identify and participate in process improvement project
    Tech-in-Charge
    Required to perform the duties of the tech-in-charge for specified periods. Oversee operations according to departmental guidelines. Understands when to defer issues to appropriate personnel, for example, a supervisor or director, and does so.
    Report Reviewing and Results Reporting
    Reviews lab results as required and takes appropriate action before verification and release. Recognizes and calls critical results within defined time frames. Reviews paper-generated results and transcribes them accurately into the Lab Information System
