This provider will take over and maintain a panel of patients for all acute, chronic and health maintenance needs.

       

      Department/division specs

      • Practice alongside 10 primary care physicians and one fellow primary care APP
      • Primarily Family Medicine Practice seeing all ages
      • Strong access to a robust specialty referral network in the building and region

       

      Duties & Responsibilities

      • 36 patient-facing hours and four admin hours/week
      • Monday-Friday with 8-, 10- or 12-hour shift flexibility
      • Hours to include a combination of two early or two late clinics (or combo) per week
      • Up to two Saturday morning clinics/month (provider choice)
      • 4 weeks of phone-based call per year
      • 100% outpatient

       

      Preferred Qualifications:

      · Applicants with previous Advanced Practice Provider experience in primary care setting is preferred, not required. Relevant RN experience may be considered.

      Position Requirements:

      Nurse Practitioner:

      • Current licensure from the State of Illinois as a Registered Professional Nurse and Advanced Practice Nurse or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process
      • Master of Science in Nursing or Doctorate of Nursing practice with clinical specialty appropriate to position
      • Current certification by American Nurse Credentialing Center (ANCC) or American Academy of Nurse Practitioner (AANP)
      • CPR, ACLS, and PALS certifications

      Physician Assistant:

      • Current licensure from the State of Illinois as a Physician Assistant/Associate or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process

      Rush North & Harlem is significantly expanding access to Rush primary and specialty care for northwest Chicago and suburban patients in near west communities like Oak Park, River Forest and Elmwood Park, by offering some of the most sought-after healthcare services — including nationally ranked specialties like neurology and cardiology.       The Advanced Practice Provider (APP) will promote collaboration between multiple disciplines to ensure cost effective and quality patient care. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards.

      Rush offers competitive total compensation that includes base pay, benefits, and other incentive programs for our employees. This total pay range is for a full-time, 1.0 FTE employee, and includes any incentive payments that may be applicable to this role.         Summary:
        Under the leadership of the System Director of Patient Financial Clearance, the System Manager of Financial Clearance – Pre-Authorization Services provides strategic leadership and operational oversight of pre-authorization functions supporting ambulatory clinics and hospital-based services across the Rush University System for Health.         Summary:
        The Unit Clerk performs clerical duties associated with customer service, the patient care record and electronic health record on the inpatient units. The Unit Clerk is directly responsible for the maintenance of the patient care record and its contents as well as coordinating patient activities, services and supplies from ancillary and support areas that directly relate to patient care.         Responsibilities:
        1. Customer service

        1.3 Answers and screens phone calls to determine appropriate person or response needed.

        2. Patient flow/bed control

        2.2 Schedules diagnostic tests and notifies nursing staff of patient schedules on a per shift basis.
        2.3 Reconciles census every shift.
        2.4 Performs walking census reconciliation a half hour before the end of each shift.
        3. Patient medical records
        3.1 Assumes accountability for the maintenance of medical records and ensures that current records and records of previous hospitalizations are returned to the Medical Records department as described in policy.
        3.2 Processes admission, transfer, expiration, and discharge records and completes any other functions associated with patient records in a timely, accurate and legible manner.
        3.3 Maintains patient care record by insuring all forms are properly labeled with patient I.D. and incorporates them into the patient care record in a timely, organized and accurate manner.

      The Advanced Practice Provider (APP) will promote collaboration between multiple disciplines to ensure cost effective and quality patient care.         Responsibilities:
        1. Customer service
        1.1 Greets customers by establishing eye contact, smiling as appropriate and offering assistance.
        1.2 Answers incoming telephone calls promptly and courteously, states greeting, name, unit, and offer of assistance.
        1.3 Answers and screens phone calls to determine appropriate person or response needed.

        1.5 Owns complaints received from customers and follows through; if resolution is not obtained promptly, directs complaint to manager.

        2. Patient flow/bed control
        2.1 Collaborates with Patient Placement, Patient Access, Unit Clerical Manager, and Environmental Services to ensure room readiness for patient admits or transfers.
        2.2 Schedules diagnostic tests and notifies nursing staff of patient schedules on a per shift basis.
        2.3 Reconciles census every shift.
        2.4 Performs walking census reconciliation a half hour before the end of each shift.
        3. Patient medical records
        3.1 Assumes accountability for the maintenance of medical records and ensures that current records and records of previous hospitalizations are returned to the Medical Records department as described in policy.
        3.2 Processes admission, transfer, expiration, and discharge records and completes any other functions associated with patient records in a timely, accurate and legible manner.
        3.3 Maintains patient care record by insuring all forms are properly labeled with patient I.D. and incorporates them into the patient care record in a timely, organized and accurate manner.
        3.4 Follows through on all processes using appropriate unit procedures, such as Kardexes, diagnostic test sheet and patient charges.
        3.5 Initiates and follows all electronic health record downtime procedures in an accurate and timely manner.
        4. Unit operations/technology
        4.1 Utilizes complex telecommunication systems.
        4.2 Utilizes and trouble shoots all unit based communication equipment.
        4.3 Identifies and trouble shoots technology failures and handles computer down time.
        4.4 Maintains proper par levels of unit supplies and forms by re-ordering in a timely fashion.
        4.5 Maintains and utilizes unit pneumatic tube stations for clerical and clinical functions.
