      Summary:

      Summary:

    Summary:

    The Licensed Clinical Social Worker (LCSW) is a key member of the Health Promotion & Disease Prevention Team within the Department of Social Work & Community Health (SWaCH) at Rush University Medical Center. This role offers clinical support at the Sankofa Village Wellness Center (Sankofa) and is responsible for the coordination, implementation, and oversight of Rush Generations evidence-based programming for individuals with chronic illnesses and older adults.

    This position serves a dual function:

    1.On-Site Social Work Presence: Provides responsive, on-demand social work support to patients, caregivers, and community members at Sankofa, addressing social needs and crises with professionalism, cultural humility, and sound clinical judgment, including appropriate escalation when necessary.

    2.Programmatic Leadership: Oversees the planning, recruitment, coordination, delivery, and quality assurance of evidence-based group interventions (e.g., chronic disease self-management, falls prevention, emotional wellness) to ensure high-fidelity implementation and strong community engagement.

    The LCSW works closely with interprofessional teams, community partners, and Sankofa staff to ensure programming is accessible, culturally responsive, and aligned with community needs. All outreach and participant interactions are documented in the electronic medical record (Epic).

    •Master’s Degree in Social Work.
