The Division of Hematology, Oncology and Cell Therapy in the Department of Medicine at Rush University Medical Center is recruiting an Oncologist or a Hematologist/Oncologist to join its breast medical oncology faculty at the assistant, associate, or professor level. The successful candidate will be board certified in medical oncology or hematology/oncology and has demonstrated interest and academic productivity in a field of breast oncology. Summary:
Clinical pharmacy services will be provided to the malignant hematology, medical oncology, gynecologic oncology, and stem cell therapy (SCT) recipients and/or the family/caregivers of SCT recipients during inpatient hospitalization for cancer, chemotherapy and its complications and all phases of SCT. Clinical pharmacy services will be provided in collaboration with pediatric pharmacists for pediatric oncology. b.Participate in development of patient education materials with multidisciplinary team members in preparing patients for SCT.
c.Be available on a consultative basis to the outpatient SCT clinic on patients with immunosuppression problems identified by any multidisciplinary transplant team member.
Inpatient Acute Care:
1.The hematology, oncology, gyne/onc and SCT pharmacists:
a.Attends interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology and SCT daily to assist the respective team with the design, implementation and monitoring of medication regimens.
b.Reviews drug therapy to ensure appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, drug interactions and cost-effective therapy.
c.Reconciles home medications for admitted patients.
d.Discusses medication order clarifications with the prescriber/healthcare team and document any changes in patient and pharmacy records to inform others of medication order changes.
e.Provides individual patient/family counseling on medication regimens, adherence to medication regimens and adverse effects of medication regimens to patients/caregivers prior to discharge of allogeneic SCT patients and other patients deemed appropriate and document in the electronic medical record.
f.Provides pharmacokinetic consultations as required. Kinetic notes are written for every new start of vancomycin or an aminoglycoside and for dose modifications.
g.Prevents, detects monitors, documents and reports adverse drug reactions and medication errors.
h.Reviews chemotherapy orders for hematology, oncology, gyne/onc, and SCT patients receiving chemotherapy inpatient.
i.Available to nursing staff for questions on medication orders.
j.Order verification/in-basket messages for assigned units.
k.PM staffing required, frequency based on departmental need.

Discharge Planning (Inpatient Acute Care):

b.Help facilitate discharge planning for smooth transition to outpatient pharmacies or alternate site of care.
c.Assist in assessing insurance coverage and assist in prior authorization for medications (when necessary) in advance of discharge.

Summary:
The Organ Placement Clinical Coordinator is a registered nurse trained in the evaluation of deceased donor organ offers, waitlist review and patient selection, donor-recipient matching, informed consent procedures for organ offers and acceptance, and all aspects of the coordination, planning, and follow-up on organ offers, procurement, and transplantation procedures. Ability to apply knowledge of the United Network for Organ Sharing (UNOS)/ Organ Procurement and Transplantation Network (OPTN), the Centers for Medicare and Medicaid Services (CMS) Condition of Participation (CoP) for Transplant Programs, and the Joint Commission governing organ donation and transplantation in the U.S. is required to ensure hospital and transplant program adherence to all regulations.

•Presents critical aspects of the organ offers to the Transplant Surgeon focusing on donor history, hospital course, medications and major treatment interventions, laboratory review including serology, and blood type. Requires clarifying communication between the offering Organ Procurement Organization (OPO) and the OPTN to obtain complete clinical, psychosocial, infectious disease, laboratory, and logistical information on the organ donor and specific organ offer.
Provides advanced patient selection assistance to the transplant surgeon and assists in reviewing, screening, selecting, and recommending transplant candidates based on organ specific criteria. Coordinates the transplant event process which includes: patient hospital admission set up, clinical team notification to include surgery, OR, on-call resident, ICU Team, Blood Bank, transplant histology lab, Infectious Disease, etc. Coordinates start times of organ recovery and organ transplant.
•Detailed documentation of all communications and decisions are required on a case worksheet. Coordinates the recovery of the organ to include transportation logistics (ground and air), timing, and retrieval team selection including Surgeon, Preservationist, and Fellow.
•Reviews organ specific information that may impact patient decision making, provides education and answers questions, and obtains patients informed consent to accept a specific offer understanding the risks-benefits. Performs a nursing assessment of potential candidates to determine readiness and medical, psychosocial, and financial suitability to undergo transplantation at time of organ offer. Provides guidance to the transplant team on donor-recipient matching criteria such as infectious disease screening, and other factors critical to the patient pre and perioperative care plan.

The Division of Hematology, Oncology and Cell Therapy in the Department of Medicine at Rush University Medical Center is recruiting an Oncologist or a Hematologist/Oncologist to join its breast medical oncology faculty at the assistant, associate, or professor level. The successful candidate will be board certified in medical oncology or hematology/oncology and has demonstrated interest and academic productivity in a field of breast oncology. Summary:
Clinical pharmacy services will be provided to the malignant hematology, medical oncology, gynecologic oncology, and stem cell therapy (SCT) recipients and/or the family/caregivers of SCT recipients during inpatient hospitalization for cancer, chemotherapy and its complications and all phases of SCT. Clinical pharmacy services will be provided in collaboration with pediatric pharmacists for pediatric oncology. The hematology/oncology/SCT pharmacy service will include identifying, solving and preventing medication related problems in the hematology/oncology/SCT population. Inpatient Acute Care:
1.The hematology, oncology, gyne/onc and SCT pharmacists:
a.Attends interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology and SCT daily to assist the respective team with the design, implementation and monitoring of medication regimens.
b.Reviews drug therapy to ensure appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, drug interactions and cost-effective therapy.
c.Reconciles home medications for admitted patients.
d.Discusses medication order clarifications with the prescriber/healthcare team and document any changes in patient and pharmacy records to inform others of medication order changes.
e.Provides individual patient/family counseling on medication regimens, adherence to medication regimens and adverse effects of medication regimens to patients/caregivers prior to discharge of allogeneic SCT patients and other patients deemed appropriate and document in the electronic medical record.
f.Provides pharmacokinetic consultations as required. Kinetic notes are written for every new start of vancomycin or an aminoglycoside and for dose modifications.
g.Prevents, detects monitors, documents and reports adverse drug reactions and medication errors.
h.Reviews chemotherapy orders for hematology, oncology, gyne/onc, and SCT patients receiving chemotherapy inpatient.
i.Available to nursing staff for questions on medication orders.

•Autonomously presents the patients' candidacy to the multi-disciplinary team, prepares the patients information for candidacy to medical and surgical team discussion and conscientious for listing patient on the national organ transplant waitlist to receive organ offers.
•Performs on call responsibilities for those patients’ awaiting transplantation.
•Arranges for and communicates about patient admission and history to appropriate team members.
•Shares on-call and weekend responsibilities as assigned, in order to assure Transplant Coordinator coverage after hours and on weekends.
•Reviews and interprets the organ match run to identify the appropriate Rush transplant candidates.
•Presents critical aspects of the organ offers to the Transplant Surgeon focusing on donor history, hospital course, medications and major treatment interventions, laboratory review including serology, and blood type. Requires clarifying communication between the offering Organ Procurement Organization (OPO) and the OPTN to obtain complete clinical, psychosocial, infectious disease, laboratory, and logistical information on the organ donor and specific organ offer. Provides advanced patient selection assistance to the transplant surgeon and assists in reviewing, screening, selecting, and recommending transplant candidates based on organ specific criteria.
•Coordinates the transplant event process which includes patient hospital admission set up, clinical team notification to include surgery, OR, ER, AICU Team, security, etc. Coordinates start times of organ recovery and organ transplant. Coordinates the recovery of the organ to include transportation logistics (ground and air), timing, and retrieval.
•Conducts post-discharge planning and education for transplant patients.
•Compiles and reviews post-discharge data for clinical management of transplant patients.
•Provides telephone consultation and follow-up to support patients and their families.
•Maintains patient status in EMR and UNOS based on guidelines established by UNOS and CMS.
•Assesses, plans, and evaluates interventions for patients in the clinic or remotely.
•Collaborates in the development of protocols and guidelines for patient management.
•Develops, implements, and evaluates comprehensive patient education programs.
•Participates in the development and execution of quality assurance programs and projects.
•Ensures that the Rush Transplant Program remains in compliance with the National Organ Procurement and Transplant Network (OPTN), established by the Federal Government.
•May serve as a peer-to-peer coach for onboarding new team members and transplant coordinators.
•Supports planning of and participates in clinical research projects.
•Forms the nursing process (assessment, care planning, intervention, evaluation) independently and with high level of expertise within the limits of licensure and scope of practice for patients with various states of end stage organ disease and transplantation.
•Provides population management for case load of 100-200 patients.
•Follows Rush University Medical Center and Solid Organ Transplant Program policies, procedures, techniques, and guidelines.
•Creates and maintains appropriate documentation for transplant candidacy and post-transplant follow-up care in accordance with CMS and UNOS requirements, in required timelines.
•Provides required education across the continuum of the transplant process appropriate to the phase of care to transplant candidates, families, and significant others in accordance with CMS and UNOS requirements.
•Reports listing of patients on the waitlist, changes of patient status, time of transplant and follow-up via TIEDI in required timeframes.
•Ensures completion of all transplant evaluation requirements (diagnostic, laboratory and consultative) to be presented at weekly Selection Conferences.
•Prepares, presents, and effectively communicates patient evaluation findings to a multidisciplinary team of physicians, surgeons, and ancillary services at weekly Selection Meetings to determine transplant candidacy in accordance with the Solid Organ Transplant Program policies and procedures.
•Participates in survey-readiness and understands and complies with regulatory requirements of planning, regulatory, inspecting agencies, and solid organ transplant policies (Medicare Conditions of Participation; Department of Health Services; Joint Commission; United Network for Organ Sharing).
•Completion of TIEDI form to meet CMS requirements of greater than or equal to 95% completion within specified time frame across the continuum of care.
•Demonstrates ability to dialogue with surveyors effectively. Actively participates in CMS Transplant focused and UNOS surveys, during interviews and patient tracer audits providing evidence to show regulatory requirements are being meet.

Summary:
The Organ Placement Clinical Coordinator is a registered nurse trained in the evaluation of deceased donor organ offers, waitlist review and patient selection, donor-recipient matching, informed consent procedures for organ offers and acceptance, and all aspects of the coordination, planning, and follow-up on organ offers, procurement, and transplantation procedures. Ability to apply knowledge of the United Network for Organ Sharing (UNOS)/ Organ Procurement and Transplantation Network (OPTN), the Centers for Medicare and Medicaid Services (CMS) Condition of Participation (CoP) for Transplant Programs, and the Joint Commission governing organ donation and transplantation in the U.S. is required to ensure hospital and transplant program adherence to all regulations.

Responsibilities:
Receives all organ offers through the OPTN/UNOS Donor Net Online Portal. Requires disposition of organ offer to include an acceptance or turn down with refusal codes within 30 minutes of electronic offer.
•Reviews and interprets the organ match run to identify the appropriate Rush recipients, identify when to select recipients from local versus national match-run, identify when offer must by-pass Rush or when Rush has secured local back-up. Understands and communicates effectively waiver information on each import organ, understanding the type of waiver and implications for payment of the organ.
•Presents critical aspects of the organ offers to the Transplant Surgeon focusing on donor history, hospital course, medications and major treatment interventions, laboratory review including serology, and blood type. Requires clarifying communication between the offering Organ Procurement Organization (OPO) and the OPTN to obtain complete clinical, psychosocial, infectious disease, laboratory, and logistical information on the organ donor and specific organ offer.
Provides advanced patient selection assistance to the transplant surgeon and assists in reviewing, screening, selecting, and recommending transplant candidates based on organ specific criteria. •Detailed documentation of all communications and decisions are required on a case worksheet. Coordinates the recovery of the organ to include transportation logistics (ground and air), timing, and retrieval team selection including Surgeon, Preservationist, and Fellow.
•Reviews organ specific information that may impact patient decision making, provides education and answers questions, and obtains patients informed consent to accept a specific offer understanding the risks-benefits. Performs a nursing assessment of potential candidates to determine readiness and medical, psychosocial, and financial suitability to undergo transplantation at time of organ offer. Provides guidance to the transplant team on donor-recipient matching criteria such as infectious disease screening, and other factors critical to the patient pre and perioperative care plan.
•Add patients to the OPTN/UNOS organ transplant waiting list at the request of the Transplant Physician.
Performs nursing assessment by phone and provides directions to patients. Consults with MD/ARNP as clinical protocols determine. Insures pertinent data from transplant event are recorded accurately in the database.
•Will cross train to provide organ preservation coverage as needed to cover transplant center volumes Participates in quality assessment and performance improvement activities including but not limited to data entry, data abstraction, data analysis, report compilation, QAPI Committee presentations, participation in improvement teams, and waitlist management.
