      Pay Range: $29.36 - $47.79 per hour
      Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. The pay range for each role reflects Rush’s anticipated wage or salary reasonably expected to be offered for the position. Offers may vary depending on the circumstances of each case.

      Summary:
      The Licensed Clinical Professional Counselor provides outpatient care for patients engaged with Rush MD Anderson Cancer Center at Rush Lisle. The Counselor provides patient care through a diverse role as a clinician, care coordinator, and advocate. The Counselor manages a caseload of oncology patients by providing psychosocial assessments and cares management support. Duties include performing biopsychosocial assessments, providing care management services, addressing resource needs, providing patient/caregiver & staff education, and providing crisis interventions when needed. This position is part of the Supportive Oncology team, a multidisciplinary team supporting the Rush Cancer Center, and the Counselor works in collaboration with other healthcare team members to meet patients¡¦ psychosocial needs. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

      Responsibilities:
      •Manages a caseload of Rush Supportive Oncology patients to determine needs.
      •Provides telephonic and in-person outreach to support patients and their families.
      •Conducts biopsychosocial assessments in order to identify patients¡¦ needs and develop a plan of care.
      •Develops a psychosocial plan of care and facilitate clinical interventions to meet patients¡¦ needs.
      •Demonstrates ownership of patients¡¦ care plans, and evaluate progress based on clinical judgment and ongoing assessment of patient’s needs.
      •Provides targeted counseling designed to help patients and caregivers cope throughout the course of cancer treatment and survivorship
      •Maintains a caseload of patient referrals from Rush cancer clinics
      •Completes timely documentation via the electronic medical record
      •Remains knowledgeable of evidence-based practices in counseling
      •Refers patients to other mental healthcare providers or other supportive oncology providers as necessary
      •Coordinates via formal or informal case consultation with other Supportive Oncology providers
      •Problem-solves with patients, their health care team, and other providers as needed regarding psychosocial needs and care coordination.
      •Promotes the Rush Supportive Oncology program, helping clients to connect with programs that address their support needs.
      •Documents all patient/caregiver interactions in Epic electronic health records system.
      •Represents the Supportive Oncology department at meetings, conferences, or other public engagements on an as-needed basis.
      •Other duties as needed

      Other information:
      Required Job Qualifications:
      • Master’s degree in Counseling or Social Work required.
      • Current State of Illinois Social Work or LCPC licensure required.
      • At least two years of experience in healthcare or social services as a licensed social worker.
      • Strong training and experience providing evidence-based clinical interventions.
      • Knowledge of community resources and clinical expertise related to psychosocial issues, care management, and crisis management.
      • Experience working with adult oncology patients in a medical setting preferred.
      • Familiarity with electronic medical records preferred.
      • Proficiency with Microsoft Office.
      • Self-directed and possess ability to perform tasks independently, problem-solve, and prioritize and manage workload.
      • Strong active listening, critical thinking, judgment and decision-making skills.
      • Strong interpersonal, teamwork and organizational skills.
      • Strong written and verbal communication skills.
      • Maintain licensure and meet ongoing CEU requirements by attendance at professional development meetings, seminars, workshops. Willingness to present information to peers, team, etc.
